Appointments
If you would like to receive care at our clinic, please ask your primary care provider for a referral. Returning patients can schedule an appointment through their MyChart account.
New patients
We ask that new patients arrive at least 15 minutes before for their appointment. By arriving early, you will have ample time to fill out necessary paperwork.
What to bring
- Your insurance card
- Your photo identification, such as a driver’s license
- A list of the supplements and medications you currently take
- A form of payment
