Schedule your appointment by calling our clinic during business hours. If you are a returning patient, you can also use your MyChart account to schedule your follow-up visit.
New patients
If this will be your first time visiting us, you can call to schedule your appointment. Depending on your insurance plan, you may need a referral from your primary care provider. Please plan to arrive at least 15 minutes early to your appointment each time you visit us.
What to bring
- A form of photo ID
- Your insurance information
- A list of the medications and supplements you currently take
- A method of payment to cover your copayment
