You can schedule a visit with one of our neurological providers by calling our clinic during our normal office hours. Please have information about your health insurance plan available when you call so we can help you determine coverage for your visit.
What to expect
You will receive a reminder call from our clinic one to two days before your appointment. If you need to reschedule or cancel your appointment, please call our clinic 24 hours in advance. If you call after our normal business hours, your call will be answered by our after-hours service.
What to bring
Please bring your insurance information/cards and a form of identification, as well as a list of your current prescription and over-the-counter medications, including herbal or nutritional supplements. Please include information about doses and frequency of use so that our team has a good understanding of your medication routine.
Please arrive 15 minutes prior to your scheduled appointment to ensure all paperwork is complete. Please note a referral is required to make an appointment.
