Appointments
If you would like to make an appointment at our office, please call us during our operating hours. If you are a returning patient, you can also schedule a visit on MyChart using your computer or smartphone.
If you are a new patient, please come in 15 minutes early for your appointment. This will give you a chance to fill out needed paperwork.
What to bring
- Your insurance card, or the insurance card that covers your child
- Your photo identification
- A method of copayment or full payment, if insurance will not cover this visit
- The medications you or your children are currently taking, in the original containers if possible
