Appointments

You can schedule an appointment by calling our clinic during our normal office hours, or through your MyChart account. Please have your health insurance plan information available when you call so we can help you determine coverage for your visit. A physician referral is required to make an appointment.

You will receive a reminder call from our clinic one to two days before your visit. If you need to reschedule or cancel, please call our clinic 24 hours in advance. If you call after our normal business hours, your call will be answered by our after-hours service.

What to bring

In addition to a caregiver or guardian, please bring your insurance cards and form of identification, as well as a list of your current prescriptions and over-the-counter medications, including herbal or nutritional supplements. Please include information about doses so that our team has a good understanding of your medication routine.

New patients

Please arrive 15 minutes prior to your scheduled appointment to complete all paperwork.
