Appointments

To schedule a visit for yourself or your child at our clinic, please call our office to make the appointment. If you’ve been to our clinic before, you can schedule an appointment through your MyChart account.

Please arrive at least 15 minutes prior to your appointment or your child’s appointment. This will give you time to complete important paperwork before you see the doctor.

What to bring

• Your photo identification
• Your insurance card, or the insurance card that covers your child
• A list of the medications you take regularly, or that your child takes regularly
• A method of payment to cover the cost of your visit or your child’s visit
