Appointments
To make an appointment with a provider in our clinic, you can use MyChart or open scheduling through our provider page to schedule online right now. You can also call our clinic during office hours.
If you would like to receive an appointment reminder by text message instead of a phone call, please give your mobile phone number to a front desk team member. Once prompted, you'll need to text “Novant” to 622622 to sign up.
We look forward to seeing you in our office. Please arrive at least 15 minutes before your first appointment. Bring with you:
What to bring
- Your photo ID
- Your insurance card, or the insurance card covering your child if your child is the patient
- A payment to cover your copay or payment in full, if you will be responsible for costs of the visit
- A list of any medications you are or your child is currently taking
- Forms you have completed ahead of time using the links below
