Appointments

If you would like to make an appointment at our clinic, please call our office during business hours. If your primary care provider or another provider on your care team has submitted a referral to our office, we will reach out to you to schedule your visit.

If this will be your first time at our office, we ask that you arrive 20 minutes early so you can fill out necessary paperwork before your appointment.

What to bring

• A list of the medications you regularly take, including vitamins
• A photo ID card
• A health insurance card
• A prescription card
• A method of payment, if insurance will not cover the entire cost of your visit
