- Call during business hours to plan your visit.
- If you need to reschedule or cancel your appointment, call us at least 24 hours in advance.
What to bring
- Your vitamins and supplements
- Records of your medical and surgical history
- Insurance card
- Copay and payment toward your deductible
New patients
We are pleased that you have decided to receive care at Novant Health Cancer Institute - Forsyth (Integrative Medicine). Before calling to schedule an appointment:
- Contact your primary care provider and ask for a referral to our office. A referral is not required but can assist with transfer of your health records.
- Contact your insurance company and verify coverage.
- Please arrive at least 30 minutes early for your first visit. By arriving early, you will have time to fill out required forms.
