Appointments
To schedule a visit at one of our clinics for a second opinion, risk assessment or genetic counseling session, please ask a member of your care team for a referral.
If this will be your first time visiting us, please try to arrive 15 minutes early. This will give you time to complete important forms before your appointment begins.
What to bring
- Photo ID
- A form of payment, if insurance will not cover the total cost of your visit
- Your insurance card
- A list of the medications you take regularly, including supplements
