Appointments
To schedule a visit at our clinic, you can call our office during business hours. You do not need a referral from to be seen by our providers. If you do have a referral from a primary care provider or specialist, please let us know when you call.
Returning patients can also use their MyChart account to make an appointment.
New patients
If this is your first time with us, we ask that you arrive 30 minutes before your scheduled visit. This will give you enough time to fill out needed paperwork before you see your provider.
What to bring
- Your photo ID
- A list of the medications you are currently taking
- Your insurance coverage information
