Appointments
Please call us during business hours to make an appointment. Your insurance company may require you to have a referral in order to pay for your visit. If that is the case, please contact your primary care provider and ask for a referral to our office. Returning Novant Health patients can also schedule a visit with us on their computer or smartphone using MyChart.
New patients
If this is your first time visiting our clinic, please arrive at least 15 minutes early so you can fill out important paperwork.
What to bring
- Photo ID
- Copayment, or full payment if this visit will not be covered by insurance
- Insurance card, if you have one
- Recent scans or X-rays of your spine
- A list of medications you are currently taking, including dosages
