Appointments
Please call us during business hours if you would like to make an appointment with our office. You can also use the scheduling feature on Mychart using your smartphone or computer.
New Patients
Please arrive at least 15 minutes early for your appointment. This will give you enough time to fill out necessary paperwork.
What to bring
- Photo ID
- Insurance card
- A way to cover your copayment, or full payment if you do not have insurance that will pay for this visit
- A list of the names and dosages of the medications you are currently taking
