Appointments
To schedule an appointment with a provider in our clinic, use MyChart to schedule online anytime. You can also use open scheduling through our physician finder, or call our clinic during office hours.
If you are sick or injured and would like to be seen quickly, call our office so we can serve you with a same-day or next-day appointment if possible.
What to bring
We welcome new patients. Please arrive at least 15 minutes before your appointment time, and bring with you:
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- A list of any medications you are currently taking
- Any forms you have completed ahead of time using the links below
