Diabetes education center program

Our diabetes education program is provided to teach you how to self-manage your diabetes. Since there is no cure for diabetes, self-management is crucial to maintaining a healthy life. We will meet with you and create a management plan that will give you the knowledge and treatment tools you need to live a happy, independent life.

After you are referred to us by your primary care physician, we will schedule an initial appointment and follow-up classes or individual appointments, depending on your needs. This program covers all the basics of diabetes management.

  • Pre-diabetes or metabolic syndrome education - Topics include understanding health risks associated with diabetes, basic meal planning skills, carbohydrate counting and portion control, label reading and finding and following an appropriate exercise plan.

  • Diabetes education - Sessions include use of medications (oral or insulin), self-monitoring of blood glucose, advanced carbohydrate counting, pattern management, proper foot, skin and dental care, avoiding and treating hypoglycemia / hyperglycemia, sick day rules and much more.

  • Gestational diabetes education - A registered nurse or registered dietitian provides information for expectant mothers about gestational diabetes. This includes facts about gestational diabetes, healthy eating, home blood glucose monitoring, being active, and insulin administration (if needed).

The American Diabetes Association recognized our Diabetes Education Center for quality in self-management.

Diabetes support group

We also offer a diabetes support group to provide support and education to you and your family. This group generally meets on the first Tuesday of every other month at 6 p.m. in the hospital cafeteria. Guest speakers present a variety of topics to keep you up-to-date on diabetes care. Meetings are free and open to the public.
